29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:  a) 18691 General Puller Hwy, Deltaville, VA 23043: On or about January 10, 2019, and times prior thereto, the Baldor bench grinder was not equipped with a top tongue guard that could be adjusted to the constantly decreasing diameter of the abrasive wheel, exposing employees to struck-by hazards.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1915.71(j)(1): Scaffolding, staging, runways, or working platforms which are supported or suspended more than 5 feet above a solid surface, or at any distance above water, were not provided with a railing, which had a top rail whose upper surface was from 42 to 45 inches:  a) 18691 General Puller Hwy, Deltaville, VA 23043, M/V Grace: On or about January 10, 2019, and times prior thereto, the employer provided a fabricated frame scaffold that did not have a top rail in frame openings that were not used for access to the first or second tier platforms, up to approximately 5'2" above the lower level, where employees performed fiberglass repair work, exposing employees to fall hazards.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
