Location: 56-58 Elm St, Newark, NJ.  a.) Employees were exposed to amputations and other hazardous conditions when utilizing a power hand tool that had an abrasive wheel attached while not being equipped with the hand tool's protective guards; on or about 1/4/19.

Location: 56-58 Elm St., Newark, NJ.  a) An employees involved in masonry operations at the job site was working from a tubular welded scaffold that was not fully planked at the working level.  The employee was exposed to a potential fall hazard of up to approximately 25 ft. to the lower level; on or about 1/4/19.
