29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Jobsite: Employees were working on a roof 7 to 8 feet from ground level, removing and closing up window openings, without the use of a fall protection system.

29 CFR 1926.50(b): Provisions were not made prior to commencement of the project for prompt medical attention in case of serious injury: Jobsite: On 12-19-18, Prompt medical attention was not made available when an employee suffered a serious laceration while operating a power saw. An ambulance was not called to the job site to provide medical care and transportation to the hospital. Injured employee was first driven to residence and then ambulance was called.
