The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. Location - 78 Kirkland St. Cambridge, MA On or about 9/1/2022, employees were exposed to falls injuries while utilizing lifelines connected directly to the front/waist positioning ring and not using D-rings located at the center of back.

The Z & R Speciality Work LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1370568, citation number 1, item number 1 and was affirmed as a final order on 2/27/2019, with respect to a workplace located at 1550 River St., Hyde Park, MA 02081.
