On January 9. 2019 employees were exposed to fall hazards, in that after hearing something fall to the ground while descending the tubular welded scaffold, a competent person did not conduct an adequate inspection when the structural integrity of the scaffold was affected being that the pins did not stay in place to support the end gates structural integrity.
