29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a.) On or about March 25, 2025, the employer failed to ensure that the horizontal lifeline used by employees installing roof panels was properly installed. The employer anchored a Class B FallTech 727650C SRL at roof level instead of overhead, contrary to manufacturer's instructions, resulting in an unapproved leading-edge setup. As a result, employees were exposed to an 11 ft 8 in fall hazard to the ground below.

29 CFR 1926.502(h)(1)(v):The safety monitor shall not have other responsibilities which could take the monitor's attention from the monitoring function. a) On or about March 25, 2025, the employer failed to ensure that the designated safety monitor was free from other responsibilities while employees installed insulated roof panels on a low-slope roof. The foreman assigned as the safety monitor was also required to perform roofing tasks, assist employees with material handling, and at times observed from the ground, interrupting continuous monitoring of workers on the roof. As a result, employees working near the roof edge were not effectively monitored and were exposed to an 11 ft 8 in fall hazard.
