29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  a.   On or about January 10, 2019, on the roof and in the trusses, the employer did not provide stairways or ladders for employees to access the roof while installing trusses.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.   Employees working at elevations greater than 6 feet from the ground or lower levels while performing commercial framing are not being protected from fall hazards by a fall protection system.  The employer fails to assure effective fall protection against falls as required by the standard as most recently demonstrated on:   1.  On or about January 10, 2019, on the 6:12 pitch roof, seven framing employees installing trusses and truss bracing were exposed to a 10 feet fall hazard from the lower eaves in that they were not protected by a fall protection system  Doris L. Enamorado Lazo dba New Coast Construction and its Successors was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1309790, Willful citation number 02, item number 001 and was affirmed as a final order on July 25, 2018, with respect to a workplace located at 15 Jennie Lake Rd, Lot 58, Saint Augustine, Florida.  AJ New Construction and Repair, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1193504, Willful citation 1, item 1, and was affirmed as a final order on March 16, 2017, with respect to a workplace at 4072 Coastal Avenue, Jacksonville Beach, Florida.
