Extension cord used for skill saw had ground plug removed from male end, creating a grounding hazard to employees. 29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws      The employer is failing to protect the employees from flexible cords as they entered the plug were showing signs of strain relief in that the insulation had been pulled away from the plug. Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws.  Extension cord supplying power to skill saw had strain relief pulled back from plug in.    This was most recently observed at or about 3924 E Street in Elkhorn, Ne 68022 on January 11, 2019.
