29 CFR 1926.501(a)(2): The employer shall determine if the walking/working surfaces on which its employees are to work have the strength and structural integrity to support employees safely. Employees shall be allowed to work on those surfaces only when the surfaces have the requisite strength and structural integrity. (a) Employee(s) performing work on a flat awning roof next to exposed edges on three (3) sides were not protected from falling to the lower level. The work surface had not been determined to have the strength, or structural integrity required to support employees properly during demolition of the awning roof. This exposed the employee(s) to a fall hazard of approximately 12 feet.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: (a) At the North side of the building, the personal fall arrest systems (safety harnesses) being worn by two employees working from an awning, had not been rigged to prevent a fall greater than six (6) feet. This exposed the employees to a fall distance of approximately 12 feet.
