(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: a. On or about January 7, 2019, in the injection area, the employer failed to ensure that employees who perform activities on top of injection molding machines such as clearing/cleaning out solidified nylon plastic material and Asaclean from the feed throat and barrel on the I33 Haitian injection molding machine (model: MA6000; S/N: 201107060034995) were protected from falls 4 feet or more above a lower level. On or about January 7, 2019, an employee performing the aforementioned task was exposed to a fall hazard of approximately 6 feet, 1/16 inch and was forced away and onto the ground when liquefied nylon, gaseous, and/or other material was released through the feed throat due to pressure buildup.

On or about January 7, 2019, an employee performing the aforementioned task received burns on the face, neck, and arm from liquefied nylon, gaseous, and/or other material when it was released through the feed throat due to pressure buildup.

29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance. a. On or about January 7, 2019, in the injection area, the employer failed to ensure that their lockout/tagout procedure was utilized while employees were clearing/cleaning out solidified nylon plastic material and Asaclean from the feed throat and barrel after the feed hopper was removed on the I33 Haitian injection molding machine (model: MA6000; S/N: 201107060034995), thereby exposing employees to a struck-by and caught-in hazard.

The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or about January 7, 2019, in the injection area, the employer failed to ensure that employees who were clearing/cleaning out solidified nylon plastic material and Asaclean from the feed throat and barrel after the feed hopper was removed on the I33 Haitian injection molding machine (model: MA6000; S/N: 201107060034995) had the knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy; perform an orderly shutdown by completely shutting down the injection molding machine; isolate the injection molding machine from the energy sources; affix lockout or tagout devices to each energy isolating device; relieve, disconnect, restrain, and otherwise render safe all potentially hazardous stored or residual energy for the injection molding machine; and verify that isolation and deenergization of the injection molding machine had been accomplished, thereby exposing employees to a struck-by and caught-in hazard.
