29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  a) Smyrna Ready Mix Concrete, LLC, Gainesville, GA - On or about 01/11/19, and times prior to, employees performing driving and loading of trucks with rocks and concrete was exposed to noise at a level of 93 dBA to 123 dBA. The employer had not conducted monitoring to evaluate employee's 8 hour TWA exposures.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:  a) Smyrna Ready Mix Concrete, LLC, Gainesville, GA - On or about 01/11/19, and times prior to, employees of the corporation performing duties such as, but not limited to, loading and/or unloading trucks, were exposed to dust, sand, and debris were not provided and/or required to wear protective eye equipment.    b) Smyrna Ready Mix Concrete, LLC, Gainesville, GA - On or about 01/11/19, and times prior to, employees of the corporation performing duties such as, but not limited to, loading and/or unloading trucks, were exposed to dust, sand, and debris were wearing sunglasses that did not comply with ANSI Z.87.1, American National Standard Practice for Occupational and Educational Eye and Face Protection.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
