29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields: On or about January 25, 2019, where employees were welding during the manufacturing of aluminum trailers, the employer did not ensure that employees walking on the shop floor were adequately protected from rays by barriers including, but not limited to welding curtains.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: On or about January 25, 2019, employees that had been permitted to wear 3M 6200 Half Mask Air Purifying Respirators while welding had not first been provided a medical evaluation for respirator use. MAC Trailer Manufacturing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(c)(2)(ii), which was contained in OSHA inspection number 1310502, citation number 2, item number 1, and was affirmed as a final order on August 21, 2018, with respect to a workplace located at 1453 Allen Road, Salem, OH.
