29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent: a. 3200 Shawnee Industrial Way, Suite 100, Suwanee, GA (Front Plant Area) - Employees were exposed to entrapment hazard. There were designated, marked, and identified emergency exit doors located throughout the shop area were hidden by the large moving machineries, moving conveyors and other large equipment, and were not in the line of sight. There were no directional exit markings indicating the way to travel, on or about 01/24/2019.

29 CFR 1910.144(a)(1)(iii): Emergency stop bars on hazardous machines such as rubber mills, wire blocks, flat work ironers, etc., were not red. Stop buttons or electrical switches which letters or other markings appear, used for emergency stopping of machinery were not red: Or in the Alternative: 29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. a. 3200 Shawnee Industrial Way, Suite 100, Suwanee, GA (Press Machine #7) - Employees were exposed to struck-by and caught-between hazard. The "red" emergency stop button on the press machine/line #7 was missing and/or broken off from the electrical switches housed onto the machine #7, on or about 01/24/2019.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: a. 3200 Shawnee Industrial Way, Suite 100, Suwanee, GA (Shop Area) - Employees were exposed to sudden energization or start-up of multi-energy source machines. The employer did not conduct periodic inspections of the Lockout/Tagout procedures to assure that the procedures were being followed and corrections were made as needed to the Press machines and other multi-energy source machines, on or about 01/24/2019.
