29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures: a) 2806 Longshore Ave, Philadelphia, PA - The employer did not ensure that undermined concrete sidewalk was braced and shored prior to letting employees work in the trench thus exposing employees to struck-by hazards. This conditioned was observed on or about January 17, 2019.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. a) 2806 Longshore Ave, Philadelphia, PA - The employer failed to keep excavated material at least two feet from the edge of the trench thus exposing employees working in the trench to cave-in hazards. This condition was noted on or about January 17, 2019. The QQ Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(j)(2), which was contained in OSHA inspection number 1070159, citation number 01, item number 003 and was affirmed as a final order on March 1, 2016, with respect to a workplace located at 1501 Porter St, Philadelphia, PA 19145.
