Instance b. Slitting lathe Machine - On or about January 15, 2019, the rotating shaft was not guarded to prevent employees from contact with a rotating part. Instance c. KBC Lathe Machine - On or about January 15, 2019, the KBC (model Grip-1540 S/N 700450) lathe machine's lead screw was not guarded to prevent employees accidental contact with the rotating part.
