29 CFR 1960.8(a): The head of each agency shall furnish to each employee employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm. a) On or about 01-18-2019 and times prior thereto, located at FCC Petersburgh, Federal Bureau of Prisons at 1060 River Road Hopewell, VA 23803, Condition: Untrained and unqualified inmate employee(s) were exposed to 220 volt electrical shock and falls from elevated working ladder surfaces while attempting to repair an inoperative LED light fixture. Inmate was not prevented from obtaining, using, or falling from a 10 foot ladder, after coming in contact with electrical source. Ladder was classified as a Class A tool, as per Federal Bureau of Prisons FCC Petersburgh security protocol, and inmate was not authorized to be in possession of tool. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
