Condition: Employee(s) were exposed to work areas that did not have an illuminated exit sign. The mounted emergency exit sign located above the main entrance/exit door was inoperable. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

Condition: Employee(s) were exposed to electrical hazards from a damaged Dayton pedestal fan. The fan's electric motor had been damaged and re-wired and was relying on cloth and packaging tape to hold the fan's parts together. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
