NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATMENT CERTIFICATION FOR THIS ITEM FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
