29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. On or about 28 January 2019, and at times prior thereto, a portable fire extinguisher within the office was blocked by various objects, preventing safe and ready access, exposing employees in the office to a tripping hazard.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. On or about 28 January 2019, and at times prior thereto, a platform attachment was being used by two employees within the warehouse without the written approval of the powered industrial truck manufacturer, exposing the employees to the hazard of being struck by the basket or ejected from the forklift due to tipping. The platform attachment was not secured to the forklift other than the slots on the platform, was not rated for the capacity or stability for elevated positions, and was not designed, tested, and approved by an engineer.
