On or about 28 January 2019, and at times prior thereto, a platform attachment was being used by two employees within the warehouse without the written approval of the powered industrial truck manufacturer, exposing the employees to the hazard of being struck by the basket or ejected from the forklift due to tipping. The platform attachment was not secured to the forklift other than the slots on the platform, was not rated for the capacity or stability for elevated positions, and was not designed, tested, and approved by an engineer.
