29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a) New residential construction located on Lot #29, Lower front roof: On or about 01/28/2019, and at times prior, the employer exposed employees to struck-by hazards, in that protective eyewear was not provided to employees operating pneumatic siding nail guns at 120 PSI while installing eave, rake, and soffit boards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) New residential construction located on Lot #29, Lower front roof: On or about 01/28/2019, and at times prior, the employer exposed employees to fall hazards of 15-feet, 4-inches to the lower level, in that a fall protection system was not provided or used to prevent falls to the lower level as employees were engaged in cornice board and trim work.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  a) New residential construction located on Lot #29, Front side of home at cut bench: On or about 01/28/2019, and at times prior, the employer failed to inspect all of the electrical cords prior to the start of the work shift. The strain relief on the Makita Circular Saw was damaged and was not removed from service or repaired.
