29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or before January 29, 2019, the employer did not establish an energy control program consisting of energy control procedures, employee training, and periodic inspections to ensure that prior to an employee performing servicing and maintenance; such as but not limited to retrieving bowling balls, resetting pins for the 32 pin setting machines, and working on the turret, deck, and rake to protect employees exposed to moving and energized parts.

29 CFR 1910.305(d)(2): Panel boards were not mounted in cabinets, cutout boxes, or enclosures designed for the purpose and/or were not dead front: On or about January 29, 2019, the main electrical panel board, located in the rear of the bowling alley behind the pinsetters, had no dead front on it thus exposing employees to electrical hazards. There was also water dripping directly in front of the electrical panel.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panel board enclosures in damp or wet locations were not installed to prevent moisture or water from entering and accumulating within the enclosures: On or about January 29, 2019, the main electrical panel board, located in the rear of the bowling alley behind the pinsetters, was not installed to prevent water from entering the enclosure thus exposing employees to electrical hazards.
