29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:     a) Martel Construction, Inc./Cabinet Shop Spray Area: On or about January 29, 2019, and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; Acetone, n-Butyl Acetate, and Formaldehyde (Sher-Wood Fast Dry Vinyl Sealer) while using these chemicals in various phases of the wood treatment process. The employer did not perform medical evaluations for employees required to use the respirators.        b) Martel Construction, Inc./Cabinet Shop Spray Area: On or about January 29, 2019, and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; Acetone, n-Butyl Acetate, and Formaldehyde (Sher-Wood Fast Dry Vinyl Sealer) while using these chemicals in various phases of the wood treatment process. The employer did not conduct fit testing for employees required to use the respirators.     c) Martel Construction, Inc./Cabinet Shop Spray Area: On or about January 29, 2019, and at times prior, employees were exposed to respiratory hazards from chemicals such as, but not limited to; Acetone, n-Butyl Acetate, and Formaldehyde (Sher-Wood Fast Dry Vinyl Sealer) while using these chemicals in various phases of the wood treatment process. The employer did not ensure that employees were trained on the limitations of respirator cartridges and filters.
