29 CFR  1903.19(c)(1): Within 10 calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section.   On or about 8/20/2019, the employer failed to certify to OSHA that the following hazards have been abated.  (a) Production, Polishing/grinding area-  On or about February 5, 2019, and at times prior, the employer exposed an employee to noise hazards, in that the employer failed to establish an audiometric testing program for an employee exposed to noise levels above the Action Level of 85 dBA while polishing and/or grinding metal parts.    An employee polishing and/or grinding metal parts was exposed to continuous noise at 70.89 % of the permissible daily exposure of 85 dBA (time weighted average) or an equivalent sound level of 87.5 dBA during the 435 minute sampling period on February 5, 2019. The exposure calculation included a zero increment for the 45 minutes not sampled.  b) Wood Shop- On or about February 5, 2019, and at times prior, the employer exposed an adhesive sprayer to a health hazard, in that the employer did not provide training on the hazards associated with methylene chloride.  Commercial Stainless Fabricators, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.1052(l)(1), which was contained in OSHA inspection number 1100435, citation number 1, item number 4 and was affirmed as a final order on April 5, 2016, with respect to a workplace located at 1035 Cobb Industrial Drive, Marietta, Georgia 30066.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  (a) Production, Polishing/grinding area-  On or about February 5, 2019, and at times prior, the employer exposed an employee to noise hazards, in that the employer failed to establish an audiometric testing program for an employee exposed to noise levels above the Action Level of 85 dBA while polishing and/or grinding metal parts. 29 CFR 1910.1052(l)(1): The employer did not provide information and training for each affected employee prior to or at the time of initial assignment to a job involving potential exposure to methylene chloride:  a) Wood Shop- On or about February 5, 2019, and at times prior, the employer exposed an adhesive sprayer to a health hazard, in that the employer did not provide training on the hazards associated with methylene chloride. 29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  (a) Wood Shop, Adhesive spraying area- On or about February 5, 2019, and at times prior, the employer exposed an employee to a respiratory hazard, in that the employee required to wear a tight-fitting face piece type respirator while spraying adhesive, had not received a medical evaluation to determine his ability to safely wear the respirator.
