Storage of material created a hazard: On or about January 31, 2019, and at times prior thereto, the employer did not ensure that bales of recycled material located throughout the production area were stacked, blocked, interlocked or limited in height so they were stable and secure against sliding and collapse. Employees were exposed to the hazards of being crushed-by and/or struck-by, associated with falling bales of recycled material.
