On or about December 9, 2020, and at times prior thereto, at the Republic Truck Shop Yard, portable jacks were not being inspected prior to use, such as but not limited to, employees removing a spider gear from the axle of Truck 3612, used a portable jack that was not marked and was missing all of its saddle adapters.
