29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: "AND AS REFERENCED BY THE NOTE CONTAINED IN 1926.602(d)" a) Jobsite - On or about and at times prior to January 30, 2019, the employer did not ensure an employee who operated a JLG Rough Terrain Telehandler forklift (Model JLG 6042, S/N 0160050999), was properly trained and re-evaluated, thus exposing employees to possible unsafe operations of powered industrial trucks hazards.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles. a) Jobsite- On or about January 30, 2019, an employee was allowed to operate a Masonry Saw, Model MP25E1, S/N MK5505106, to cut concrete block without the use of eye protection, exposing employees to eye injuries from ricocheting and flying debris.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) Tubular Welded Scaffolding System-On or about January 30, 2019, two employees were working from tubular welded frame scaffold without a ladder or any other means of safe access/egress to reach the upper levels of the scaffold. Employees were observed using crossbracing as a means of access, thus exposing employees to a fall hazard up approximately 14 feet. 1. Southeast Scaffold Section - 14 feet 2. Southwest Scaffold Section- 4 feet The Willie J. Harswell dba Willie J. Harswell Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)1), which was contained in OSHA inspection number 1254789, citation number 1, item number 1 and was affirmed as a final order on October 3, 2017, with respect to a workplace located at 649 West James St, Savannah, GA 31401.
