29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached:  a) 60 Salem Rd., Longmeadow, MA: On or about January 28, 2019, two employees were attached to reusable roof anchors that were not properly attached to the roof structure, rendering them not capable of supporting at least 5,000 LBS per employee attached.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).

JCL Home Improvement Inc was previously cited for a violation of this occupational safety and health standard 29 CFR 1926 503 (b) 1, which was contained in OSHA inspection number 1306128, citation number 1, item number 2 and was affirmed as a final order on July 5, 2018, with respect to a workplace located at 68 Hopkins Pl., Longmeadow, MA.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
