29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: On or about 07 February 2019, the employer did not document hazardous energy control procedures for use by employees assigned to perform servicing or maintenance tasks on the production equipment to include the SampSistemi Buncher, stranders, welders, and all other production machines. Employees were exposed to unexpected start-up (energizing) likely to cause fractures and other injuries associated with caught-in and struck-by hazards. Abatement documentation is required for this item, only 2 procedures have been supplied for review.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition shall be removed from service. All repairs shall be made by authorized personnel. On or about 07 February 2019, the employer allowed the Mitsubishi #1 SN AF13F10208 powered industrial vehicle to remain in service with tires that were visibly damaged and in need of repair. This exposed employee to struck-by hazards while traveling or being passed by other vehicles if the vehicle lost control. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. On or about 07 February 2019, the employer failed to ensure that all powered industrial trucks were properly inspected and removed from service if any safety conditions were found. On the Mitsubishi #1 SN AF13F10208 Powered Industrial Vehicle, all four of the tires were bald with no tire tread and were severely damaged with chunks of rubber missing. This exposed the employee who were operating the PIV and also all employees who were in the vicinity to a struck by hazard if the PIV lost control. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

On or about 07 February 2019, the electrical disconnect switches used to ensure the complete deenergization of the machines when conducting Lockout/Tagout operations were broken on the following machines. a.
