29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    (a) West, rewinding department, Bartell rewinder no. 3 (serial No. TU72-898-LH): on or about June 3, 2019, operators assigned to guide a winding cable by a gloved hand onto a rotating wooden reel had their fingers and hands exposed to ingoing nip points and caught-in hazards likely to result in serious injury because safeguards were not provided to prevent access to the danger zone.    (b) West, rewinding department, Bartell rewinder no. 3 (serial No. TU72-898-LH): on or about June 3, operators assigned to guide a winding cable by a gloved hand onto a rotating wooden reel had their fingers and hands exposed to a rotating reel's clamp caught-in hazard that was likely to result in serious injury because safeguards were not provided to prevent access to the danger zone.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours.    West, rewinding department, Bartell rewinder No. 3: on or about June 3, 2019, a work-related incident that resulted in an amputation was not reported timely to OSHA within twenty-four (24) hours, either in person to the OSHA Area Office that is nearest to the site of incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov.    There is no abatement certification or documentation required for this item.

29 CFR 1910.22(d)(1): (1) Walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition;  West, rewinding department, Bartell rewinder No. 3: on or about June 17, 2019, operators walking by and standing at the mounting/dismounting side of the reel winding station's were exposed to slip, trip, and fall hazards due to damaged concrete floor where uneven grooved tracks were indented (recessed).    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
