29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. On or about February 9, 2019 at 3831 W Mason Street in Oneida, employer did not ensure employees had been evaluated at least once every three years in powered industrial truck operator performance. Forklift operators did not have an evaluation since May 1, 2014. The S & M Tool, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(4)(iii) which was contained in OSHA inspection number 967509, citation number 02, item number 001, and was affirmed as a final order on July 11, 2014, with respect to a workplace located at 3831 W Mason Street; Oneida, WI 54155.
