29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about February 5, 2019, at Lot 229, 73 Maple Brook Drive, Dawsonville, Georgia:  Four employees performing siding work were exposed to struck-by hazards from falling tools and materials from the employees working from a pump jack scaffold when they worked beneath those employees while not wearing a hard hat.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about February 5, 2019, at Lot 229, 73 Maple Brook Drive, Dawsonville, Georgia:  Two employees performing siding work on a pump jack scaffold were exposed to a fall hazard of approximately 20-feet when they worked from the work platform of a pump jack scaffold without using fall protection.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  On or about February 5, 2019, at Lot 229, 73 Maple Brook Drive, Dawsonville, Georgia:  Four employees performing siding work were exposed to struck-by hazards from nails, wood chips, and siding material while operating nail guns without using eye protection.   Abel Robarte-Camacho was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1096605, citation number 1, item number 1, and was affirmed as a final order on October 27, 2015, with respect to a workplace located at Lot 160, Still Pine Bend, Smyrna, Georgia.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about February 5, 2019, at Lot 229, 73 Maple Brook Drive, Dawsonville, Georgia:  Two employees performing siding work were exposed to a fall hazard of approximately 20-feet when employees used an aluminum extension ladder that had not been extended three feet above the pump jack work surface landing.   Abel Robarte-Camacho was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1096605, citation number 1, item number 3, and was affirmed as a final order on October 27, 2015, with respect to a workplace located at Lot 160, Still Pine Bend, Smyrna, Georgia.
