29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by paragraph (l):  On or about February 5, 2019, an employee was allowed to operate a rough terrain fork truck prior to successfully completing the required training, exposing the employee the hazards associated with operating a powered industrial fork truck.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about February 5, 2019, employees performing residential construction operations were not protected from falls.   a. Employees staging material and framing the second floor were not protected from a fall, exposing employees to a fall of over 25 feet.   b. Employees performing residential construction activities were exposed to a guardrail that was not installed within the parameters of the standard, exposing employees to a fall of approximately 10 feet.   S & G European Framing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1203692, citation number 2, item number 1 and was affirmed as a final order on April 21, 2017, with respect to a workplace located at Lot 40/41 Dublin Way Frankfort, IL 60423.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d)
