29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:  a- 414 & 416 Parkview Avenue, Lakewood NJ: An employee was exposed to fall hazards, in excess of 12 feet to the ground surface below, when walking/working from a residential structure's entryway roof without the use of fall protection.  Condition observed on or about 02/04/19.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level:  a- 414 & 416 Parkview Avenue, Lakewood NJ: An employee was exposed to fall hazards, in excess of 12 feet to the ground surface below, when using an unsecured, portable aluminum extension ladder that was set on the barrel surface of a pitched roof.  As such this extension ladder was not set on a surface that was stable or level. Condition observed on or about 02/04/19.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a- 414 & 416 Parkview Avenue, Lakewood NJ: Two company employees were exposed to fall hazards, up to and in excess of 12 feet to the ground surface below, when using two separate aluminum extension ladders both containing visibly bent side rails and rungs.  Condition observed on or about 02/04/19.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
