29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a) At the jobsite at 218 Front Street, on or about 2/5/19: A workers used a lanyard where the slack left in the line was such that the worker would have fallen at least 6 feet and likely struck the ground if the equipment was needed. Workers also installed and used 3 different anchor points that were not installed according to the manufacturers instructions and would not have provided 5,000 lbs. of strength if the equipment was needed. Workers demonstrated a lack of training in the deployment of personal fall protection equipment.
