29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) New clubhouse construction in Serenade at Newnan Crossing Subdivision, Roof: On or about February 5, 2019, and at times prior, the employer failed to provide training to employees in the recognition of hazards related to elevated walking/working surfaces and aerial lift equipment, in that employees were not provided with the Personal Protective Equipment and training needed to perform tasks on these working surfaces and equipment.

(a) New clubhouse construction in Serenade at Newnan Crossing Subdivision, Roof: On or about February 5, 2019, and at times prior, the employer exposed employees to fall hazards of 12-feet, 11-inches to the lower level, in that a fall protection system was not provided to employees prepping and painting the cupola using the 7/12 pitched roof as a walking/working surface.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) New clubhouse construction in Serenade at Newnan Crossing Subdivision, Roof: On or about February 5, 2019, and at times prior, the employer exposed employees to ejection/fall hazards of more than 13-feet 12-feet to the lower level, in that a fall protection system was not provided to employees accessing the roof using a Genie S-60 Aerial Lift. Employees used the aerial lift to gain access to the roof to perform paint prep work on the cupola.
