29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) On or about February 5, 2019, the employer did not properly train employees to lockout/tagout equipment such as, but not limited to, the perforator. Employees did not control hazardous energy sources before they conducted activities such as, but not limited to, removing jammed material. The perforator was not isolated from energy sources prior to engaging in these unjamming activities, which exposed employees to caught in hazards.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about February 5, 2019, the employer did not ensure that employees utilized energy control procedures on the perforator machine while conducting unjamming procedures. The perforator was not isolated from the electrical energy source prior to engaging in these activities. Employees were exposed to hazards from the rollers and other moving parts while all hazardous energy sources were not controlled prior to conducting sanitation work. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)], are not implemented to protect employees from machine servicing hazards such as, but not limited to, unjamming product from rollers.
