29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid:    a) Facility Wide: On or about February 7, 2019 and times prior, employees working  with equipment such as mill presses, drill presses, press brakes, and lathes, without an infirmary, clinic or hospital in near proximity to the workplace, were not provided with a person onsite who is trained in first aid.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Bridgeport Mill Press: On or about February 7, 2019 and times prior, employees used a Bridgeport mill press to cut key ways and cut a part flat.   The rotating bit was not provided with a guard to prevent employee exposure to the flying chips and rotating parts.     b) Okk Mill press: On or about February 7, 2019 and times prior, employees used a Okk mill press to cut key ways and cut a part flat.   The rotating bit was not provided with a guard to prevent employee exposure to the flying chips and rotating parts.     c) Cincinnati Bickford Drill Press: On or about February 7, 2019 and times prior, employees used a Cincinnati Bickford Drill Press to drill holes in machine parts.  The rotating drill bit was not provided with a guard to prevent employee exposure to the flying chips and rotating parts.     d) Majestic Drill Press: On or about February 7, 2019 and times prior, employees used a Majestic Drill Press to drill holes in machine parts.  The rotating drill bit was not provided with a guard to prevent employee exposure to the flying chips and rotating parts.     e)Brideport Drill Press: On or about February 7, 2019 and times prior, employees used a Bridgeport Drill Press to drill holes in machine parts.  The rotating drill bit was not provided with a guard to prevent employee exposure to the flying chips and rotating parts.     f) US Industrial Machine Lathe: On or about February 7, 2019, employees used a US Industrial Machine lathe to manufacture machine parts. The rotating chuck was not provided with a guard to prevent employee exposure to the rotating parts.     g)Leblond Lathe: On or about February 7, 2019, employees used a Leblond lathe to manufacture machine parts. The rotating chuck was not provided with a guard to prevent employee exposure to the rotating parts.     h) Economaster lathe: On or about February 7, 2019, employees used a Economaster lathe to manufacture machine parts. The rotating chuck was not provided with a guard to prevent employee exposure to the rotating parts.     i) All Steel Piranha Press Brake: On or about February 7, 2019, employees used a All Steel Piranha Press Brake to bend machine parts. The press brake was not adequately guarded to prevent employee exposure to the moving parts.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) On or about February 7, 2019 and times prior, employees perform servicing and maintenance activities such as equipment repairs.  The employer did not perform annual periodic inspections of the energy control program to ensure the requirements of the energy control standards are met.
