29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:      On or about February 26, 2019, at this electric motor and parts manufacturing establishment, employees were exposed to noise hazards without a continuing and effective hearing conservation program in place:    a. The Machine Operator at Punch Press #1 was exposed to a noise level of 89.10 dB during a 459.26 minutes sampling period;    b. The Machine Operator at Punch Press  #2 was exposed to a noise level of 88.10 dB during a 430.58 minutes sampling period; and,    c. The Machine Operator at Punch Press  #5 was exposed to a noise level of 90.20 dB during a 416.00 minutes sampling period.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      On or about January 12, 2019, at this establishment, in the plant production area, employees were exposed to struck-by, caught-in, amputation and electrical hazards when performing service and/or maintenance on presses, such as but not limited to; removing damaged parts, repair work, and set-up. An energy control program with all required elements was not established by the employer.
