29 CFR 1910.106(d)(3)(ii)(a): The door of the metal storage cabinet(s) for flammable and combustible liquids was not provided with a three point lock: (a) The three point locking handle on the flammable storage cabinet in the paint department was missing, leaving a hole in the door and the cabinet was held closed by a barrel bolt on the top of the right door. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

(a) On February 27, 2019, the painter was observed wearing a 3M 5200 tight fitting face piece respirator with the head straps over top of a ball cap while spray painting in the paint booth with Bariline Express HAPs Free Red Base(irritation of skin, eyes and respiratory tract) and Rusteoleum PVTLBL SSPR OPP Touchstone GLS Black (irritation of skin, eyes and respiratory tract, possible allergic reactions.) In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
