29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. a) On or about 2/12/2019 the employer did not ensure that the racking systems (located in the middle of the facility) was properly stacked, blocked, interlocked and limited in height to prevent products from falling through the racks. A heavy tractor attachment sitting on a pallet that was not properly supported, in that the ply wood that the pallet was sitting on was bowing due to the weight of the attachment.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) On or about 2/12/2019 the employer did not provide adequate guarding of the two Schenck Rotor Balancing machines (Model #HAO5UVB/5 1974 and Serial # AHF 0597 1981). The gates to the Balancing machines were not interlocked allowing employees to open the gates while the machine was in operation, exposing employees to a caught in hazard. The openings for the gates (8" X 14") were large enough to allow parts to came through while the machines were in operation, exposing employees to a struck by hazard.

29 CFR 1910.219(d)(1): Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes. a)On or about 2/12/2019 the employer did not ensure that a power driven motor with a belt and pulley system (located in the Rotor Assembly & Balancing Area)was unguarded, exposing employees to a caught in hazard.
