A) Norplas Industries Inc. on or about February 11th, 2019, at the location of 7825 Caple Blvd., Northwood, OH: The employer did not provide refresher training to employees including an evaluation of the effectiveness of that training when the operator was assigned to operate the Raymond (Mod# 772-R45TT) fork truck. Due to this condition, employees were exposed to hazards such as struck by, struck against, and caught between hazards due to the lack of refresher training to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely.
