29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:   A)     Norplas Industries, inc., located at 7825 Caple Boulevard, Northwood, OH 43619: On or about March 11, the employer did not ensure that employees were protected from fall hazards while walking and working on the top of the molds/tools of the hydraulic 1300 Ton KraussMaffei Press # 0 (Machine type: KM 1300/08100/MX; Machine # 61022666). Employees conducted activities such as, but not limited to, changing molds/tools inside the press. Employees were not protected by guardrails, safety net systems, or by personal fall arrest systems. Employees were exposed to fall hazards of approximately 6 feet from the molds/tools to the platform below.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  A)     Norplas Industries, Inc., located at 7825 Caple Boulevard, Northwood, OH 43619: On or about February 17, the employer did not adequately train employees to lockout/tagout the hydraulic 1300 Ton KraussMaffei Press # 0 (Machine type: KM 1300/08100/MX; Machine # 61022666). Employees did not control all hazardous energy sources such as, but not limited to, electrical and hydraulic prior to conducting activities such as, but not limited to, changing molds/tools inside the press. Employees were exposed to struck-by and caught-in-between hazards while performing this task.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):   A)     Norplas Industries, Inc., located at 7825 Caple Boulevard, Northwood, OH 43619: On or about February 17, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed. Employees did not deenergize the hydraulic 1300 Ton KraussMaffei Press # 0 (Machine type: KM 1300/08100/MX; Machine # 61022666) prior to performing activities such as, but not limited to, changing molds/tools inside the press. Employees were exposed to struck-by and caught-in-between hazards while performing this task.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from struck-by and caught-in-between hazards while employees conducted activities such as, but not limited to, changing molds/tools inside the KraussMaffei Press # 0.
