29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) Facility, 2658 Puckety Street, Export, PA: On or about February 11, 2019 the employer had not conducted a periodic inspection of an energy control procedure to ensure the procedure was adequate and met the requirements of the standard. The employer's inspection protocol consisted of an employee looking at the list of equipment that had multiple energy sources and telling them if anything had been updated; the employee did not take a procedure and follow it to ensure the steps adequately controlled all energy sources for the item being serviced.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: (a) Facility, 2658 Puckety Street, Export, PA: On or about February 11, 2019 authorized persons who perform servicing and maintenance on equipment had not been trained on or shown the specific energy control procedures to ensure they were aware of energy sources, type and magnitude and methods necessary to isolate such sources.
