29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a.    On or about February 12, 2019, on the 5:12 pitch second-story roof, an employee re-installing roofing paper was exposed to 22 feet to 27 feet fall hazards from the eaves and raked edges of the roof in that he was not protected by a fall protection system.  b.    On or about February 12, 2019, on the 5:12 pitch roof above the front porch, three employees were exposed to an 11 feet fall hazard in that they were not protected by a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.    On or about February 12, 2019, at the front porch eave, the roof access ladder to the front porch roof was not extended 3 feet above the eave, exposing employees to an 11.7 feet fall hazard.  b.    On or about February 12, 2019, on the 5:12 pitch porch roof, the roof access ladder to the second-story roof was not extended 3 feet above the roof edge of the upper landing surface, exposing an employee to a 6 feet fall hazard to the lower roof.
