OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck-by cut-off machinery and/or other objects:  a)  On or about February 13, 2019, employees were exposed to the hazard of contacting a rotating cutting blade, being struck-by thrown objects, and/or reactive forces, including kickback, where a carbide-tipped wood-cutting circular saw blade was provided and used with a Stihl TS 420 cut-off machine.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

Commercial Ave., Lake Barrington, Illinois 60010.  Employees are exposed to the hazard of falling approximately 17.5 feet to the ground, while engaged in snow and ice removal work along an unprotected edge of the roof.   To abate this hazard in the future, the employer must ensure that it's employees are protected from hazard of falling while performing roofing work along unprotected edge of the roof. Commercial Ave., Lake Barrington, Illinois 60010.  The warning lines that are erected around all open sides of the roof work area are placed approximately 40 inches from the edge of the roof.  Five Star Roofing Systems, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(f)(1)(I), which was contained in OSHA inspection number 980144, citation number 1, item number 1a and was affirmed as a final order on October 22, 2014, with respect to a workplace located at 2726 W. Lake Ave., Peoria, Illinois 61615.  To abate this hazard in the future, the employer must ensure that it's employees are protected from hazard of falling while performing roofing work by erecting warning lines at least 6 feet from the roof edge. Five Star Roofing Systems, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(h)(1)(v), which was contained in OSHA inspection number 1204382, citation number 1, item number 2 and was affirmed as a final order on May 17, 2017, with respect to a workplace located at 1 Dearborn Square, Kankakee, Illinois 60901.  To abate this hazard in the future, the employer must ensure that the safety monitor does not have other responsibilities which could take the monitor's attention from the monitoring function.
