On 2/11/19, at the Athens facility, the employer did not ensure the periodic inspection corrected deficiencies found in the specific written procedures. On Extruder 9 Die Heater Bands and Extruder 9 Extruder procedures heat energy was not listed as an energy on the specific written procedure (400-450 degrees).

29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected. On 2/11/19, at the Athens facility, the employer did not ensure all Authorized employees were included in the periodic inspection of the established Lockout program and instead the review took place between the same 2 employees for the past 6 years. Additionally, thermal energy sources were missing from the Lockout/tagout procedures. Employees can be protected from the thermal energy sources by the use of Personal Protective Equipment.
