Employees were exposed to hazardous metal fumes, gases, vapors, and radiation, as well as the fire and electrical hazards of arc welding, and they were not trained on the hazards and methods of protecting themselves, including ventilation and personal protective equipment. Employees were not trained on the specific OSHA regulations found in 1910.254(d)pertaining to:      1) hooking up equipment, e.g., checking all connections; checking for leaks of gases, cooling water, and engine    fuel; freeing magnetic work clamps from spatter; ensuring switch for shutting down equipment is operational       2) grounding arc welding equipment,       3) safe handling of welding cables and electrodes holders,       4) ensuring that manufacturer's instructions are printed and strictly followed
