29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel: At the worksite located at 1331 Civil War Road, Carthage, Missouri: Hyster forklift 1501, serial number C219N01563N, was leaking hydraulic fluid and was not taken out of service for repair. Employees were using the forklift to move product through the facility exposing employees to struck by hazards due to forklift malfunction.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in wet locations were not weatherproof: At the worksite located at 1331 Civil War Road, Carthage, Missouri: Employees were exposed to electric shock hazard at an electrical junction box adjacent to an electrical panel box in the area of rail cut F6. The electrical junction box was covered in ice on February 11 2019 and July 9, 2019.
