29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Custom Made Meals, LLC at 5575 Logan Court Denver, CO 80216: On or before February 12, 2019, the employer did not develop, document, and utilize procedures for the control of potentially hazardous energy when employees were cleaning the Batter Applicator 24 machine. The pipe elbows connected to the Batter Applicator 24 pump motor were removed, and this condition exposed employees working on the machine to the blades of the motor. An employee was cleaning the Batter Applicator 24 on the night shift without locking it out and tagging it out and/or placing it in the hygiene mode. An employee's finger was partially amputated by the blades of the motor when this employee was sanitizing the machine. Among other feasible and acceptable abatement methods to correct this hazard is to ensure that all employees and supervisors follow the manufacturer's procedures of placing the Batter Applicator 24 in hygiene mode when sanitizing the equipment.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: a) Custom Made Meals, LLC at 5575 Logan Court Denver, CO 80216: On or before February 12, 2019, an employee was exposed to caught in-between and amputation hazards when sanitizing the Batter Applicator 24 machine. The night shift sanitation crew was not trained on how to properly de-energize the Batter Applicator 24 machine when cleaning the machine. An employee was assigned to clean the Batter Applicator 24 machine without the use of proper lockout tag out and without engaging the machine in the hygiene mode. An employees finger was partially amputated by the blades of the motor when he was sanitizing the machine.
