29 CFR 1915.89(c)(2): The employer did not ensure the use of locks or a tags-plus system to prevent energization or startup, or the release of hazardous energy, before any servicing was started. a) On or about 02-18-2019 and times prior thereto, located at Norfolk Naval Shipyard; Code-930 Building 171 and 31 Machine Shop, Portsmouth, VA 23702, Condition: Employee(s) were exposed to potential mechanical hazardous energy while changing chuck lathes without lock-out/tag-out controls in place. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
